Who can use the Center?

Any City of Mesa employee and family member who are currently enrolled in the active employee health plan and eligible COBRA health plan members.

Can retirees or retiree families use the Center?

No.

What if I do not have active employee health insurance?
Is the Center still available to me?

No.   At this time the City is offering this benefit to active employees enrolled in the health plan and COBRA eligible members only. Center staff will validate employment and benefit enrollment at the time of service.

Do you accept Medicare or Medicaid?

No.

Do my labs drawn at the center count toward my deductible?

Yes.  All labs drawn in the center, and sent out to our in-network lab provider, will count toward your deductible.

Is the Center able to obtain the results of my blood work from my previous provider?

Yes.   Please stop by the center to obtain a release of medical information form for the practice to reach out to your current clinic to transfer your medical records.

Does the center have onsite radiology services?

No.    The center staff will refer you to an in-network radiology site to obtain your studies.

Can we still go to our current doctor?

Yes.   However, the cost of your care will be more expensive.

Are walk-in appointments accepted?

With our on-line scheduling system, we recommend that you see what appointments are available and then call the center for a possible same day appointment. Patients with previously scheduled appointments do have priority over call-ins. We will make every effort to take care of your immediate needs.

How do I schedule an appointment?

First, register as a new patient to the online Healthcare portal through the link on the home page. It may take up to 24 hours for this registration as they set up your account and respond to you.  Following this initial registration, you may then login as an existing patient and schedule a new appointment anytime from the convenience and privacy of your home or work.  Under New Appointment, select the Facility (Mesa Health and Wellness) and then select the doctor or provider.  Finally, select the type of visit you are requesting.  The calendar is then available for you to choose the date and timeframes (morning, afternoon, anytime) and the available appointment times are displayed.  You can also call the center at 480-644-WELL (9355).  PLEASE NOTE:  Each benefitted family member will need to register.

Are preventative services covered at the Wellness Center?

The center providers can provide well woman and well man physicals, along with sports, school and camp physicals.

Will the City of Mesa have access to my protected health care information?

No.  The City of Mesa has contracted with OnSite Care, Inc. as the third party vendor to provide medical services in the City of Mesa Health and Wellness Center.  Federal law - the Health Insurance Portability and Accountability Act (HIPAA) - protects the privacy of personal health information collected by health care providers, health plans, hospitals and clinics. The City of Mesa will not and does not have access to any of your medical records or records of any treatment you may receive at the Center.
